Osteoporosis and Bone Health in Aging India

It doesn’t announce itself with pain or a dramatic symptom. Osteoporosis works quietly, thinning bone density year after year, until a minor fall, stepping off a curb, slipping in the bathroom, ends in a fracture that changes everything. As India’s population ages, this “silent epidemic” is fast becoming one of the country’s most pressing, and most under-discussed, orthopaedic challenges.

Why India is especially vulnerable

Several factors converge to make bone health a uniquely Indian problem. Vitamin D deficiency is widespread despite abundant sunshine, largely due to lifestyle factors, indoor working hours, pollution, skin covering, and low dietary intake of fortified foods. Calcium intake across much of the population also falls well below recommended levels, rooted in dietary patterns that don’t prioritize dairy or calcium-rich foods.

Add to this India’s rapidly aging demographic. As life expectancy rises, more people are living long enough for age-related bone loss to become clinically significant, and postmenopausal women, who lose bone density rapidly after estrogen decline, are especially at risk.

The fracture domino effect

A hip fracture in an elderly patient is rarely just a hip fracture. It often triggers a cascade: prolonged immobility, loss of independence, increased risk of pneumonia and blood clots, and in many cases, a permanent decline in quality of life. For a country where multigenerational households still rely on elders remaining active and involved, an osteoporotic fracture can reshape an entire family’s routine.

The awareness gap

Unlike diabetes or hypertension, bone density isn’t something most Indians ever get tested for, bone mineral density (DEXA) scans remain underutilized, often only ordered after a fracture has already occurred, rather than as preventive screening. Many patients only learn they have osteoporosis when it’s already advanced.

What orthopaedic specialists are pushing for

  • Earlier screening 
  • Vitamin D and calcium supplementation
  • Weight-bearing exercise and strength training
  • Fracture Liaison Services (FLS)
  • Public health messaging

A call for proactive care

Osteoporosis isn’t an inevitable cost of aging, it’s a manageable, often preventable condition when caught early. As India’s elderly population grows, the country’s orthopaedic community faces a choice: keep treating fractures after they happen, or start building the screening and awareness infrastructure to prevent them in the first place. The doctors leading that shift deserve as much recognition as those performing the surgeries.
