Few professions carry the special privilege that obstetrics does, the chance to be present at the exact moment a family becomes whole. It’s a specialty defined not by managing illness, but by witnessing life begin, a distinction that shapes everything from the pressure obstetricians carry into every delivery room to the quiet, lasting reward that keeps them returning to it, year after year, delivery after delivery. For Dr. Kalyani Singh, Chairman of Lord Buddha Koshi Medical College and Hospital, that reward has never dimmed, even after decades in the field.

It’s this quality, the ability to find fresh meaning in a moment repeated thousands of times over a career, that placed Dr. Kalyani Singh firmly among India’s top gynecologists. “There have been countless memorable cases throughout my long career,” she notes, “but what has touched me the most deeply are the enduring words of gratitude from my patients.” It’s a reminder that has stayed with her throughout her career, that the deepest professional fulfillment often comes not from complexity, but from the simplest, most human exchange between doctor and patient.

Against the Odds

Dr. Kalyani Singh came of age in Bihar where educating daughters was still the exception rather than the norm. Through the 1960s and into the early ’70s, society offered women just two paths considered respectable – teaching or medicine, and medicine, by unspoken convention, meant becoming a “lady doctor” devoted specifically to women’s ailments. That narrow social script is what steered her toward Obstetrics and Gynaecology in the first place.

In 1972, she enrolled at Darbhanga Medical College, at a time when Bihar itself was struggling, gripped by poverty and barely functioning infrastructure. A government scholarship helped, but never quite enough, as she ended up cooking her own meals through all of undergraduate life just to make ends meet. It was there, in those lean and demanding years, that she built the discipline and self-reliance she’d lean on for everything that followed.

Years of Service, the Seeds of a Dream

Her first posting after internship and residency took her deep into rural service alongside her husband. Dr. Kalyani Singh was assigned to the Rajmahal Primary Health Centre, a remote outpost in what was then South Bihar and is today part of Jharkhand. Even amid that posting, she kept building her credentials, earning a Diploma in Obstetrics and Gynaecology from Darbhanga Medical College, followed by an MS from Patna Medical College. A transfer in 1989 brought her back to her hometown of Saharsa, where she went on to treat thousands of women over the following decade, before being posted once again to South Bihar in 2001. It was around this point that she and her late husband, Dr. P.K. Singh began envisioning something larger than either of their individual careers, a dream of building a medical institution specifically for the Kosi region. That plan was never confined to a single medical college; it eventually grew into three interconnected institutions, Lord Buddha Koshi Medical College, Lord Buddha Koshi Nursing Sansthan, and Lord Buddha Koshi Pharmacy College.

An Uphill Battle

Turning that ambition into reality was an uphill battle. Bihar’s business climate is famously unforgiving, and the couple ran into obstacles at nearly every step, the scale of investment required pushing them into bank loans of close to INR 15 crore. Staffing proved just as hard, with specialists reluctant to relocate to a region short on infrastructure and opportunity, making it a daily struggle to meet National Medical Commission compliance while short-handed. The venture ultimately demanded more than they’d budgeted for, and when repayments fell behind, the Medical Council of India suspended admissions under court order, pushing their home, nursing home, and the college itself onto the auction block. It remains the darkest chapter of their lives. Her late husband, Dr. P.K. Singh, drove the recovery at the cost of his own health, until the institutions stood firm again; support from private investors eventually turned things around, and admissions resumed in 2019. Her elder son, Dr. Shubham Kumar, an IIT Delhi alumnus with a PhD from Barcelona, has since stepped in as Executive Director to carry the vision forward.

The Human Side of Diagnosis

Apart from being a showrunner of a medical legacy, Dr. Kalyani Singh at her core remains a true doctor. Her approach to patient care rests on a simple conviction, where a diagnosis carries psychological weight as much as physical risk, and both need tending to. “A doctor must treat not only the physical disease but also the psychological fear associated with it,” she says. When a serious diagnosis is on the table, she starts not with the patient but with the family. She walks them through the diagnosis in full, lays out every treatment option, and answers questions with what she calls “absolute honesty,” making sure they grasp both the benefits and the risks before she ever speaks to the patient directly. Only then does she turn to the patient herself, deliberately calm and reassuring in tone. “Truthful communication, when combined with genuine empathy and hope,” she notes, “empowers patients to face even the most difficult medical situations with resilience and confidence.”

Duty Over Reward

Away from the hospital and the boardroom, her interests run simpler, be it books, travel, and even social service. Charity, for her, extends well beyond free medical treatment, stretching to clothes and blankets distributed through harsh winters and financial support for underprivileged students pursuing their education. “These activities are essential for me,” she says, “they refresh my mind and continually broaden my perspective on life.”

For young doctors entering the field, her advice carries the weight of someone who has lived every part of it. She traces her own guiding philosophy to a single verse from the Bhagavad Gita, “Karmanye vadhikaraste ma phaleshu kadachana,” the idea that one has a right to duty, never to its rewards. Sincerity, honesty, and compassion, she believes, are what ultimately bring success and recognition, not the pursuit of them directly.
