The Maternal Mental Health Crisis India Isn’t Talking About

For every five babies born in India, one mother goes on to experience postpartum depression, a scale of prevalence that puts it among the most common, and most overlooked, health complications of childbirth in the country.

A large meta-analysis pooling 62 studies and over 21,000 women estimated India’s postpartum depression (PPD) prevalence at 23.5 percent, well above the 10 to 20 percent range typically reported globally. Regional studies vary widely, with some reporting figures as low as 12 percent and others crossing 40 percent, a spread researchers attribute to differences in screening tools, timing, and how openly women in different regions are willing to disclose symptoms in the first place.

What makes PPD difficult to address in India isn’t just its scale, it’s the near-total absence of infrastructure built to catch it. India’s National Mental Health Programme has existed since 1982, yet maternal mental health has never been a central focus within it. There is no routine, nationwide data collection on PPD, no standardised postnatal screening, and a chronic shortage of trained professionals. One review found only 30 percent of nurses surveyed felt confident identifying postnatal depression symptoms in patients in front of them.

Why It Goes Unnoticed

Part of the problem is cultural. New motherhood in India is framed almost entirely as a moment of joy and family celebration, leaving little room for a mother to admit she feels persistent sadness, detachment, or fear rather than fulfilment. Symptoms are often dismissed, by family and sometimes by the mother herself, as ordinary exhaustion or the emotional “ups and downs” expected after childbirth. In its more severe forms, PPD can bring overwhelming feelings of inadequacy, persistent hopelessness, and shame, the kind of symptoms that rarely get voiced in a culture where discussing mental health at all still carries stigma.

The consequences extend well past the mother. Research consistently links untreated PPD to poorer outcomes in bonding, breastfeeding, and early childhood development, meaning the cost of silence compounds across two generations rather than one.

A Shift, Slowly, in the Right Direction

There are signs of change. Maternal mental health is increasingly appearing in medical literature and public health discourse, and a small but growing number of hospitals have begun incorporating basic depression screening into postnatal visits. Awareness campaigns and more open conversations, including from mothers themselves, are chipping away at the silence that has long surrounded this condition.

What’s needed next, researchers agree, is systemic: routine screening built into every postnatal visit, better training for frontline health workers, and mental health support that’s treated as standard maternal care rather than an optional add-on.

If you or someone you know is struggling with feelings of sadness, anxiety, or detachment after childbirth, please know this is common, treatable, and not a reflection of your ability as a mother. Speaking with a doctor, gynaecologist, or mental health professional is a good first step.
