The Rising Threat of Cardio-Kidney-Metabolic Disease in Young India

The Rising Threat of Cardio-Kidney-Metabolic Disease in Young India

Ask a 25-year-old about kidney disease, heart attacks or diabetes, and most will say those are worries for their parents’ generation. The evidence increasingly says otherwise.

Doctors now recognise that the heart, the kidneys and the body’s metabolism are not separate systems that fail independently. They are closely linked, and damage to one speeds up damage to the others. In 2023, the American Heart Association gave this web a name: cardio-kidney-metabolic (CKM) syndrome. It describes how excess body fat, insulin resistance, high blood pressure and abnormal cholesterol work together to harm the heart and kidneys, usually silently and over many years.

The framework grades CKM in stages, from Stage 0, with no risk factors, to Stage 4, with established heart or kidney disease. What worries experts most is how early people are entering the early and middle stages.

Data presented in 2026 found that nearly 80% of young adults studied were already in an early or intermediate CKM stage, with excess body fat as the main driver. Kidney measures such as eGFR and creatinine worsened alongside CKM stage. Carotid ultrasound picked up detectable damage to the arteries in people who were only in their early twenties.

For India, the implications are serious. Indians tend to develop diabetes and heart disease at lower body weights and younger ages than many other populations, largely because of a tendency to store fat around the abdomen. Add long hours at desks, late nights, stress, sugary drinks, fried snacks and salt-heavy diets, and the conditions for early CKM are firmly in place. Many young Indians already carry high blood sugar, rising blood pressure or fatty liver without knowing it.

The kidneys are often the quiet casualty. Because early kidney damage causes no symptoms, a young person can lose a meaningful share of kidney function long before anything feels wrong. By the time swelling, fatigue or high creatinine appear, the damage is harder to reverse.

The encouraging part is that CKM is most reversible in its early stages. Most of what drives it can be modified:

  • Know your numbers: blood pressure, fasting sugar or HbA1c, cholesterol, waist size, and a basic kidney check (serum creatinine and a urine albumin test), especially with a family history of diabetes, heart disease or kidney disease.
  • Move daily: regular physical activity improves insulin sensitivity, blood pressure and weight together.
  • Eat simply: cut back on salt, sugar and ultra-processed food, and add more vegetables, fruit, pulses and whole grains.
  • Sleep and manage stress: poor sleep and chronic stress both push blood pressure and blood sugar upward.
  • Avoid casual painkiller use: frequent use of NSAIDs can harm the kidneys over time.

Treating CKM as one condition also changes how doctors work. A cardiologist, diabetologist and nephrologist looking at the same young patient should see one connected risk, not three separate problems.

The central lesson of CKM is simple: heart and kidney disease in your fifties often starts in your twenties. Paying attention early is how young India can avoid the illnesses its parents faced.
