C-Section - Medical Necessity or Convenience

There is a particular kind of helplessness that comes with sitting outside a labour room, watching the clock, and knowing none of it is in your hands anymore.

I felt it twelve hours in, when my wife’s delivery, which had begun with every marker of a normal, textbook pregnancy, turned into something none of us had quite prepared for. We were past the due date, contractions had been going since the previous night, and our daughter, we’d later learn, was no small baby, over four kilograms. By hour ten or eleven, the room had filled with quiet anxiety. My mother and a few relatives, watching my wife’s exhaustion mount, gently began pushing for a C-section. Then the doctor said the words that settled it for all of us – the amniotic fluid had started depleting, and waiting longer would put the baby under more stress than any of us were willing to risk.

We had a choice, technically. We chose not to wait. Given the same information today, I don’t think we’d choose differently.

I open with that story not to argue a side, but because it captures why this conversation is so difficult to have well. Our decision wasn’t convenience, it was caution and clinical judgment. Yet I’m aware thousands of Indian families make a similar call every day for reasons that have far less to do with medical urgency.

The numbers are hard to ignore. NFHS data shows C-sections now account for roughly 47 percent of institutional births in India’s private hospitals, over three times the 10 to 15 percent rate the WHO considers medically optimal at a population level. Government hospitals report closer to 14 percent. In Maharashtra, private-hospital C-section rates reportedly rose from 24 percent to 56 percent in just four years. That gap isn’t explained by riskier pregnancies alone; it points to incentives, financial and logistical, quietly nudging the decision toward surgery.

None of this means doctors are acting in bad faith. Most aren’t. Delayed childbearing, larger babies, prior C-sections, and rising patient anxiety around labour are all genuine contributors to a higher surgical baseline than a generation ago. Our own daughter’s birth sits in exactly this category – uncomplicated, until it wasn’t.

The trouble is that “necessity” and “convenience” don’t always announce themselves clearly in the moment. What I’d ask of readers navigating their own version of that labour-room clock is simple, ask your doctor what’s driving the recommendation, and what waiting would risk. A good doctor will welcome that conversation, not resist it.

Our daughter is happy, healthy, and entirely unaware of how stressful her arrival was. That’s the only outcome that mattered. I only hope more families get to make that call with the same clarity we eventually had.
