Dr. Noble Gracious S S

Dr. Noble Gracious S S

Executive Director, K-SOTTO | Professor and Head, Department of Nephrology, Government Medical College, Ernakulam

In 2000, fresh from his MD at Madras Medical College, a young doctor took charge of a one-doctor primary health centre in Ezhumattoor, a Pathanamthitta village. Alongside the clinic, he ran immunisation drives, well chlorination and mosquito control. Where some peers saw a ₹6,000-a-month posting, he saw an education in how health care reaches ordinary families. That doctor is Dr. Noble Gracious S S.

Those years taught him to look past the patient to the family. End-stage kidney disease, in his words, “does not strike one person, it strikes a household.” A Kerala study he co-authored found that nine in ten families of patients on haemodialysis faced catastrophic health expenditure.

Few nephrologists have shaped a state’s kidney care so widely. Dr. Noble helped launch Kerala’s deceased donor programme in 2012 and went on to lead the state’s organ transplant organisation as its first Executive Director. He has carried that experience into global recommendations on organ trafficking and fair access, while choosing, every time, to stay in public service.

A Coastal Village Beginning

Dr. Noble grew up in a coastal village in Thiruvananthapuram, among the first in his fishing family to receive higher education. His mother ran small shops to stretch a modest income across three children’s schooling.

He joined Government Medical College, Thiruvananthapuram, in the MBBS batch of 1988, then completed his MD at Madras Medical College, Chennai. There, Dr. Edwin Fernando, then a DM trainee under Prof. M. A. Muthusethupathi, taught him how to approach a patient with renal disease. What drew him was the specialty’s breadth. “The kidney reflects what is happening in the heart, blood vessels, immune system and metabolism, so caring for kidney patients means thinking about the whole person.”

From Diabetes to Nephrology

After four years in Ezhumattoor, the Government of Kerala posted him to the newly established Indian Institute of Diabetes in Thiruvananthapuram. There he helped develop a protocol for comprehensive diabetes care under Dr. A. K. Das and public health expert Dr. V. Mohanan Nair, fitting preparation for a specialty in which diabetes is now the leading cause of kidney failure.

He began his nephrology career at Government Medical College, Kottayam, learning critical care nephrology from Prof. Vimala and clinical warmth from Prof. K. P. Jayakumar. He completed his DM in Nephrology at Government Medical College, Thiruvananthapuram, under Prof. Ramdas Pisharody, who also shaped his public health outlook, and Prof. Jacob George, who trained him in the interventional procedures he now teaches his own fellows.

A State Programme Takes Shape

As nodal officer of the Kerala Network for Organ Sharing, and later at K-SOTTO, the Kerala State Organ and Tissue Transplant Organisation, Dr. Noble took kidney care into policy, law and programme design.

When public doubt over brain-death certification slowed donations, his team answered with transparency, strengthening certification in line with the law, opening processes to scrutiny and training doctors and coordinators. National recognition for reviving deceased donations followed, credit he shares with hospitals, the Health Department and the Kerala State Police.

His research spans pharmacogenetics-guided tacrolimus dosing and deep-learning models of liver transplant outcomes. He is also pursuing a PhD in public health at the Achutha Menon Centre for Health Science Studies, Sree Chitra Tirunal Institute for Medical Sciences and Technology.

Fourteen Years and Counting

One family has stayed with him. A boy reached end-stage kidney failure in childhood. His mother donated a kidney, but the graft failed within months, and he returned to haemodialysis and then CAPD. Soon after, his father died of a heart attack, leaving the mother with two sons and no income.

Prof. Pisharody persuaded a CAPD company to employ her as a peritoneal dialysis coordinator, a role she knew from lived experience. Her son then faced difficult vascular access and encapsulating peritoneal sclerosis needing surgery. In 2012, as the new deceased donor programme began, he received a kidney.

Fourteen years on, he lives independently as a freelance photographer. His mother still guides families through what she once faced, and his brother, helped by a Trivandrum Nephrology Club grant, is a marine engineer. “For me, this family’s journey is enough reward for being a nephrologist.”

Planning With Real Numbers

At Government Medical College, Ernakulam, where his department treats the full range of kidney disease, Dr. Noble Gracious S S aims to build a quaternary care centre offering public-sector solid-organ transplantation, with superspecialty training in nephrology and renal transplantation.

Beyond Ernakulam, his doctoral work is estimating how many people live on dialysis or with a transplant. The Government of Kerala has given in-principle approval to scale the Thiruvananthapuram dialysis registry into a statewide Keralam Dialysis Registry under the National Health Mission. Next, he wants donation after circulatory death added to Kerala’s programme.

He values virtual crossmatching for sensitised patients and sees AI as a capable assistant, if Indian patients are well represented in its data. On one point, he is firm. “The doctor still bears responsibility for every decision.”

Family, Football and Friendship

A keen football follower, Dr. Noble treats the World Cup as a welcome break. His wife, Dr. Cemmi, and their children, John and Sophia, are his anchor, and his late parents instilled the ethics that guide his decisions. His MBBS batch of 1988 remains a second family.

His advice to young doctors is practical. “Nephrology rewards people who like to think,” he says. “Learn some law, policy and statistics. They make you a better advocate for your patients.”
