In a field where cutting-edge implants and imported instrument sets often come with price tags that put them out of reach for the average Indian patient, Dr. Ramesh Chandra Thakur has built his career around a simpler, more urgent question – how do you bring the same standard of surgical excellence to a patient in Bihar as one might find in a metropolitan super-speciality hospital, without the metropolitan price tag? It is this quiet, persistent focus on accessible innovation that has made him one of the more distinctive voices among India’s orthopaedic surgeons today, and why we are featuring him alongside some of the country’s most renowned names in the specialty.
Dr. Thakur, an MS in Orthopaedics who has spent his career practising across major healthcare hubs in Bihar, is best known for developing Dr. Thakur’s Innovative Technique of Suprapatellar Nailing of Tibia, a cost-effective adaptation of one of modern trauma surgery’s more technically demanding procedures. His work doesn’t just live in journals, it lives in operation theatres where surgeons are working with limited resources but refuse to compromise on outcomes.
A Calling Rooted in Restoration
Dr. Thakur traces his path into medicine back to a fascination with its power to change lives, not just physically, but in restoring “confidence and dignity” to patients. Orthopaedic surgery, in particular, appealed to him for its immediate effects, as only a few specialties actually offer as visible a transformation as watching a patient regain mobility and independence after trauma.
That early instinct was shaped further during his training years, which he describes as teaching him that surgical skill alone isn’t enough, “successful patient care depends… on observation, discipline, ethics, and empathy.”
The Innovation That Defines His Legacy
Ask Dr. Thakur about his proudest professional milestone, and he doesn’t point to a title or a position. He points to a technique. Suprapatellar nailing has, in recent years, become the preferred approach for treating metaphyseal tibial fractures, largely because operating in a semi-extended position allows for better reduction and fixation than the older infrapatellar method. The catch – the specialized instrument sets and implants the standard approach requires are expensive, often putting the better outcome out of reach for patients in resource-limited settings.
Dr. Thakur’s answer was to redesign the process itself. His technique eliminates the need for specialized instrument sets and jigs altogether, instead adapting a protective sleeve and trocar from a standard PFNA2 set alongside a colour-coded infrapatellar nail extractor to guide the nail into position. The result is a suprapatellar approach that retains its clinical advantages while becoming dramatically more affordable to deliver.
He didn’t stop there. Building on the same philosophy, he developed the RCT Nail Introducer-cum-Positioner, a tool designed to assist in controlled, accurate nail introduction into the intramedullary canal. A comparative study of the device against the earlier extractor-based method, conducted across 25 cases each, found the newer introducer slightly reduced surgical and fluoroscopy time, while also improving accuracy during hammering.
This body of work earned him the Innovation Award from the Indian Orthopaedic Association at IOACON Guwahati 2025, the IOA’s 70th Annual National Conference, a Platinum Jubilee gathering that drew orthopaedic leaders from across the country. For Dr. Thakur, the recognition matters, but it isn’t the point. “Beyond awards and publications,” he says, “my greatest satisfaction continues to come from seeing patients recover function and return to their daily lives.”
Research With a Practical Purpose
Dr. Thakur’s published work, appearing in the International Journal of Orthopaedic Sciences, reads like a long list of problems he has solved in the field, from his original suprapatellar nailing technique and the RCT introducer study, to his experience treating 25 cases of middle-third clavicle fractures with TENS nailing, and a case report on minimally invasive management of contralateral femoral shaft and Galeazzi fractures.
His knack for continuous learning is visible in how he views the future as well. Especially when it comes to artificial intelligence’s growing role in orthopaedics, Dr. Thakur is optimistic but measured. He sees real potential in imaging analysis, surgical planning, and rehabilitation monitoring, but is clear that “technology should strengthen, not replace, the human side of medicine.”
Beyond the Operating Room
Away from the clinic, Dr. Thakur describes a life anchored in reading, mentoring younger professionals, and time with family, the kind of grounding, he says, that brings “renewed energy back into clinical work.” His philosophy on work-life balance is similarly unfussy – balance, he believes, comes not from a perfectly managed schedule but from clarity of purpose.
